Certain life events can affect your benefits. See what actions to take and what to keep in mind as you navigate life's changes. Remember, to make any changes, you must complete your enrollment and turn in any dependent certification (if applicable) through my HR within 30 days of your qualifying life event. For a complete list of qualifying life events, visit the Healthcare or Flexible Spending Account section of the Summary Plan Description. Don't wait until the last day to start! Be sure to allow yourself enough time to complete the process by the enrollment deadline.
You must make any changes to your benefits within 30 days of your qualifying life event in order to enroll on a pre-tax basis. As long as you complete your enrollment and turn in the appropriate certification within this timing, your coverage will go retroactive to the qualifying life event date. If you have a claim that must be paid before your coverage is active with the carriers, you may submit a claim form to the carrier for reimbursement once your information is active with the carriers.

- Contact myHR to request a leave, and they will walk you through your options and send you the necessary paperwork
- Notify your manager of your planned leave start date and potential return to work date
- If you are pregnant, consider enrolling in a maternity support program through your insurance carrier for personalized support
- Once the child arrives (or you are taken off of work due to a pregnancy disability), notify myHR to begin processing your leave

- Get a Social Security Number for your child

- If your return date changes or there is a change to your initial request, let myHR and your manager know as soon as possible
- Provide a return to work release from your doctor if you were disabled from work to myHR
- Access new parent resources, assistance or counseling
- Update your address if necessary

- If you’re changing your name, get a new Social Security card. Once you have a new Social Security card, contact myHR to update your name.
