Important health coverage tax documents

Form 1095-C for the 2025 tax year is available via self-service access through our Benefits Enrollment Site or upon request. Employees who have not chosen electronic delivery will no longer automatically receive a paper copy of this form by mail.

What is Form 1095-C?

This form provides information about the health insurance coverage offered to you and your family during the year, as required by the Affordable Care Act. You do not need to attach this form to your federal income tax return, but you should keep it with your tax records.

How to Request Your Form

If you would like to receive a paper copy of your Form 1095-C statement, you may request it using one of the methods below. Your form will be provided within 30 days of your request.

  • Current employees can download a copy by visiting the Benefits Enrollment Site, clicking on the “Tax Center" and selecting “Access Form" on the “Form 1095-C" tile.

  • Telephone: 877 795 2002

This notice is posted in accordance with IRS guidance and will remain available through October 15 of the year following the applicable tax year.
