Life events

You must make any changes to your benefits within 30 days of your qualifying life event in order to enroll on a pre-tax basis. As long as you complete your enrollment and turn in the appropriate certification within this timing, your coverage will go retroactive to the qualifying life event date. If you have a claim that must be paid before your coverage is active with the carriers, you may submit a claim form to the carrier for reimbursement once your information is active with the carriers.

1. Contact myHR to request a leave, and they will walk you through your options and send you the necessary paperwork
2. Notify your manager of your planned leave start date and potential return to work date
3. If you are pregnant, consider enrolling in a maternity support program through your insurance carrier for personalized support
4. Once the child arrives (or you are taken off of work due to a pregnancy disability), notify myHR to begin processing your leave
5. Change your benefits coverage and provide dependent certification (if applicable) within 30 days of the event
6. Get a Social Security Number for your child
7. Consider updating your life insurance beneficiary information
8. Consider updating your 401(k) plan beneficiary information
9. Consider updating your Health Savings Account (HSA) beneficiary information
10. Change your tax withholdings
11. Get legal support for living wills and related documents
12. If your return date changes or there is a change to your initial request, let myHR and your manager know as soon as possible
13. Provide a return to work release from your doctor if you were disabled from work to myHR
14. Access new parent resources, assistance or counseling

3. If you’re changing your name, get a new Social Security card. Once you have a new Social Security card, contact myHR to update your name.

7. Update your tax withholdings
8. Update your direct deposit, if necessary
